Athletes who are planning on participating in sports next school year, we are having our annual End of the Year Physical day on May 8th from 1-5pm located at the PAC for only $5!
If you are new to the district or sports and are an incoming Junior, you will need a new physical! You must have the medical history side filled out and signed by your parent/guardian, prior to physical day, if it is not filled out and signed, you will not be able to get your physical!
If you are an incoming Sophomore or Senior you are required to update the medical history portion only!
Physical forms will be located in S128- Athletic Training Room and with the Athletic Secretary Mrs. Alicia.
