Public Information Request

To submit a Public information Request to the Lufkin Independent School District, print out the PDF form below, complete it and mail it to the address on the form. You can also email a Public Information Request to the school district by sending an email to Sheila Adams at smadams@lufkinisd.org and including your name, email address, phone number and the information you are requesting. (Be specific about your request.)

The information you requested for copying or inspecting may be protected information under the Texas Public Information Act, Government Code, Chapter 552, which may require a determination as to confidentiality prior to its release. If you wish to receive a copy of the information requested, you may be charged 10 cents per page. Additional charges may be incurred for non standard copies or information collected on electronic medium. A deposit or bond may be required if the estimated charges exceed $100.
